Bring one concern or a short list: preparing for a move, organizing important information, or helping family understand where things stand. We use selected planner topics to focus the conversation on the work you want to do first.

The session covers an agreed scope; it does not promise completion of the entire planner. Any follow-up assistance is separately quoted.

Senior Downsizing & Relocation

Organize a move to a smaller home, a family member’s home, assisted living or a nursing home around the household’s priorities, possessions and timeline, with family participation as the client chooses.

What we plan together: A move checklist, sorting plan and agreed schedule for coordinating movers and other vendors.

Aging-at-Home Coordination

Nonmedical help organizing household tasks, service contacts and appointments around an existing care plan.

What we plan together: A household coordination checklist and contact list. Clinical and safety decisions remain with the client’s care team; construction work belongs with qualified providers.

Estate Cleanouts & Vendor Coordination

Help an authorized decision-maker organize belongings and coordinate requested sorting, donation, hauling or cleanout services.

What we plan together: A disposition checklist and vendor schedule, with decisions and approvals retained by the authorized client.

Home Inventory & Disaster Records

Help organize household photographs, item descriptions, receipts and document locations for future reference.

What we plan together: An agreed inventory format and backup checklist. An inventory does not establish value, coverage or a claim outcome.

Digital Legacy & Records Organization

Map important account categories, document locations and the people the client wants to consult about future access.

What we plan together: A records-location checklist. Clients retain passwords, recovery codes and sensitive credentials; access authority requires separate review where needed.

Annual Readiness Reviews

Revisit household changes, contact details, record locations and unfinished priorities at a separately arranged review.

What we plan together: An updated priorities list and next-step plan. Each review is individually scheduled and quoted.

Scope & Next Steps

Agree on the work before it starts.

Every service is individually quoted. The written scope identifies included tasks, expected deliverables, visits or time limits, and how additional work and third-party costs are approved. Availability and scheduling are confirmed during consultation.

We clarify who the client is, who can make decisions and which family members may receive information. Coordination does not itself authorize access to accounts, disposal of property or decisions on someone else’s behalf.

Legal, real estate brokerage and insurance services require separate engagements with the applicable professional. A readiness session or coordination plan does not create legal documents, arrange insurance coverage or provide clinical care.

Can you help organize a move into assisted living or a nursing home?

Ask about a separately quoted coordination plan for records, belongings, vendors and move tasks. The person moving and authorized decision-makers retain their choices. Clinical care needs and suitability of a facility must be discussed with the care team and provider.

Is the transition section included in the planner price?

Yes. Home, Care & Life Transitions is section 19, included in the $30 digital planner and $45 digital-and-mailed edition. Guided sessions and hands-on coordination are separately quoted.
